The process of arranging care is as follows
- We will arrange to meet you in person either in your own home or in hospital or in another location that suits you, along with any relative or professional you chose.
- We will carry out an assessment that includes talking to you about your situation and how we can help. This will help us find out what outcomes you want to achieve and support that you require
- We will assess your home situation, including any risks, so that we can keep everyone safe
- We will tell you if we can offer a service to you and if so how much it will cost
- If you want to go ahead we will write a support plan, so our Home care workers know what help to give and you know what you can expect
- We will discuss the care plan with you and finalise it with you and your family
- You will be closely matched to staff that have a similar outlook, hobbies and interests
- We will then arrange a time and date to start your care
